INSURANCE

I do not accept insurance.

If you have out-of-network benefits with your insurance plan, I can provide you with a statement of services (called a superbill) so that you may seek reimbursement from your insurance company independently.

Reimbursement is determined by your health insurance provider, so please contact your insurance company prior to treatment to determine if you have out-of-network benefits.

CANCELLATION POLICY

Patients will be charged their full session fee for appointments that are not cancelled or rescheduled with at least 24 hours' notice, or for missed appointments. The late cancellation fee will be charged to the credit card on file.
